Application Form Application Fee: DD for Rs.500- (Non-refundable application fee). Demand Draft should be drawn in favour of " Dr.Suresh Mediscan Systems Private Limited" Payable at Chennai.
Admission procedure

Before filling in the application form, the candidate should go through the prospectus carefully.
Application forms must be filled in by the Applicants in his / her own handwriting.
Incomplete application forms will be rejected.
Filled in application forms must be submitted with attested copies of the following certificates:
1. Certificates of UG (Degree or Provisional)
2. PG Diploma / PG Degree certificate
3. Registration Certificate
4. Certificates of Experience if any
5. Other qualifications if any
Originals of the above certificates must be submitted at the time of admission.

Applications will be scrutinized for eligibility and selection is made on the basis of the Registration.

Mode of payment
Fees should paid in the form of Demand Draft in favor of "Dr.Suresh Mediscan Systems Private Limited" payable at Chennai.

As soon as the selection is informed the candidate has to pay the Course fee. 50 % of the course fee will be paid soon after the selection and balance 50% will be paid on the date of joining.
  • Name and Course applied for, must be written at the back of the Demand Draft.
  • Fees once paid will not be refunded.
  • Course Fee is not refundable in case of cancellation by the selected candidates.

    General guidelines

    How to fill in application form?

  • Before filling in the application, the candidate must go through the prospectus carefully.
  • The application form should be filled-in with BLOCK CAPITAL LETTERS only, by the applicant in his / her own handwriting.
  • Name of the applicant must be written as recorded in the school / degree certificate.
  • All columns should be filled-in clearly.
  • Filled in application along with enclosures must be sent to the address mentioned above.
    All Enquiries and correspondence should be addressed to "The Director"

  • Identity Card:
    On admission, students will be issued an Identity Card by the Institution. The Identity card should be kept by the students safely till the completion of the course. In the event of the original card being lost, a duplicate card can be obtained from the Institution by remitting a fee of Rs.200/-.

    Change of Course:
    If a candidate is admitted to one course, change of course will not be permitted.

    Examinations:
    Examinations will be held at the end of the course. It will consist of Objective type questions, Hands-on, Film Reading, Biometry and Viva.

    Enrolment Number:
    Enrolment Number will be assigned to the students on admission. It should be quoted in all communications. Letters received without the enrolment number will not be entertained.
